PACE is the Program of All-Inclusive Care for the Elderly, a national healthcare program for seniors 55 and older who have multiple health issues. PACE helps seniors live in their communities instead of in nursing homes for as long as possible while improving their quality of life.

How long has Center for Elders’ Independence offered PACE?

Center for Elders’ Independence has been a PACE provider serving seniors in Alameda County for nearly 40 years and in West Contra Costa County since 2001. With the opening of the new PACE Center in Concord, CEI will now have the opportunity to bring PACE healthcare to Central and East Contra Costa County.

What type of services does PACE offer?

PACE provides complete medical, dental, vision, hearing, podiatry, transportation, home care, and social services. Medicare and Medi-Cal reimburse all care provided in the PACE program – no deductibles or co-pays.

Where is the new PACE Center located?

The center is located at 1465 Civic Court in Concord. CEI vans provide transportation for seniors to and from the center and to all their medical appointments.
