I, the undersigned, hereby acknowledge that I am the owner or authorized guardian of the dog named above. I agree to enroll my dog in the Pioneer’s Annual Woof-o-Ween Dog Costume Contest and understand that participation involves inherent risks.

I hereby release and hold harmless the Pioneer, Clayton Community Church, CBCA, and all event organizers from any claims, injuries, or damages that may arise from my participation in this event. I also agree to abide by all event rules and regulations.
